​​Instructions for completing the PHQ

The PHQ does not need to be submitted prior to testing. However, it is highly recommended to complete this document as early as possible. Contact us as soon as possible if you have any questions at recruit@vbgov.com or (757) 385-6987.

  • ​First, save a PDF copy of the Personal History Questionnaire to your computer, otherwise any information you enter may be lost
  • Instructions for completing the Personal History Questionnaire are available on page two of the document
  • After completing all four forms, have the Release of Information form notarized
  • Then, hand-deliver or mail (U.S. Postal Service, FedEx, UPS, etc.) all the forms EXCEPT the Physical Ability Test Waiver to the Law Enforcement Training Academy, 411 Integrity Way, Virginia Beach, VA 23451
  • Once your forms are received, you will be scheduled for testing
  • Save the Physical Ability Test Waiver and bring it with you on your scheduled test date
  • Packets received for a test date after the registration deadline will be considered for a later test date.

Test Dates

Applicants must complete the application process (which includes filling out and submitting both an Online Application, Credit Release Form and Authorization for Release of Information) before they can be scheduled for a VBPD Police Recruit test. Applicants will be contacted by Investigator Robert Watson to schedule a test date only after submitting all required paperwork. 

VBPD Police Academies are currently scheduled twice a year. Applicants testing on any of the listed test dates will be processed for either academy based on availability of recruit positions.
